Register yourself
Send your referral letter to info@fsdggz.nl or fill in the registration form. You will be contacted within 3 working days.
Via referrer
Your general practitioner, company doctor or medical specialist can refer you to FSD GGZ. As soon as we receive the referral letter, we will contact you within 3 days. In emergencies, we will contact you within 2 working days. The referrer can send the referral letter via the healthcare domain, or email it to info@fsdggz.nl
Your referrer can also forward the referral letter directly via ZorgDomein.
